When someone troubled by drug or alcohol addiction reaches out for help, his or her loved ones often do not know the proper steps to take to get help for the individual in need. It is also helpful during this stressful time to know what to expect when seeking help.

Step 1 : Contact UVA for a Treatment Program

Client or Family (if client is under age of 18) contacts one of our three facilities. During this phone call, a staff member will need the following information:

Name
Contact phone number
Date of Birth
ID# if applicable such as spn# or TDCJ#
Name and number of referral source

Step 2 : Schedule Appointment for Intake Assessment

At this time, the prospective client will be informed of the items needed to bring to the appointment.

Step 3 : Attend scheduled Appointment

The intake assessment takes about 1 1/2-2 hours. During this appointment, the client is asked a variety of questions regarding substance use to determine the person’s eligibility and/or treatment needs such as:

• Past & Present Chemical Use
     • Which Substances Patient is Using
     • Amount of Substances Normally Used
     • How Long They Have Been Using
     • When the Last Use Was
• Physical & Mental History
• Family Life
